Access health plan rate files and personalized cost estimates through Anthem.

Find the information you need

Anthem provides two ways to review plan pricing information: downloadable machine-readable files and a member price comparison tool.

Machine-readable files

As part of the Transparency in Coverage requirements, the Fund makes machine-readable files available on Anthem’s website. The files contain:

  • In-network rates for covered items and services
  • Allowed amounts and historical billed charges for out-of-network providers

How to find the Fund’s files

  1. Scroll to the bottom of Anthem’s search page.
  2. Enter the Fund’s EIN: 23-7003337
  3. Click “Search.”

Price comparison tool

Members can use Anthem’s internet-based price comparison tool to receive an estimate of their cost-sharing responsibility for a specific item or service from a specific provider or providers.

Ways to access the tool

  • Anthem Login Portal Access the online member tool from your Anthem account.
  • Sydney Health App Use the Anthem member experience on your mobile device.
  • Phone or paper Request the comparison information from Anthem by phone or in paper form.
